He is an extremely strong and severely demented man. He can't urinate on his own and retains urine so he has to keep the catheter. He has been to the hospital twice this week for this problem. It is not helping his confusion by repeatedly taking him to new places.

I have no personal experience, but it seems that your best hope is to keep him from having access to any part of the catheter. If he has the bag attached to his leg then an anti strip jumpsuit (yes, there are such things, just google it) would cover everything and make it difficult for him to yank on it.
