My mother's deed to her home is in her name as Trustee of the (her name) Trust. My sister wants my mom to transfer the deed to her. I have durable POA, can my sister do this without me knowing about it?

Is your mother competent to make her own decisions (from a legal standpoint)? If not, then all financial/legal decisions are your responsibility, as I understand it.

She has early stages of dementia but is medically competent. We are afraid my sister will try to talk her into transferring deed to her. Can she?

It is my understanding that as long as she is legally competent to make decisions, she can do so.
Can you sit down with her and and find out what is going on?
