My step dad lives with my moms from age 2 months old till I was 11 yrs old and was very abusive and violent to each other, after abandoning us, 48 yrs later he comes back to my life. His behavior is not good, he wants control. He has dementia where he lashed out, he starves himself for 2 or 3 days, he's diabetic. What should I need to say or do when the state comes to evaluate him so he can qualify long term facility?

Who is he living with? Is there some reason that you are still involved with this man? Usually, long term care eligibility is based on need (doctor's order) and ability to pay, assuming that he is not private pay. His doctor, who will state his needs and diagnosis, should be aware of his symptoms. Their is an application process for Medicaid andor state programs that provide for AL or Memory Care AL. You'd have to have access to his income and assets to have that completed.
