My mom has done home health 4 times. Medicare pays for it. A doctor has to order the service for the patient.

Generally Medicare will not pay for continuous care for someone with a deteriorating condition such as Alzheimer's or Parkinson's. They may pay for a few visits from therapists (occupational, physical) but will not pay for skilled nursing services or care for activities of daily living. I went through this recently, the cost to hire someone in my area was around $25/hr.
