Is your mother on Medicaid? Some states will pay a family member (not very much, apparently) to provide care at home if the person needing care meets certain eligibility requirements.

Is your mom a veteran or widow of a veteran? There are some possibilities through the VA.

Most family caregivers who are paid are paid directly by the elder himself or herself, from the savings or income that the elder has.

I have to ask, wouldn’t the sister of a 91 year-old woman be pretty old herself?

Hope you can give us some more information so we can provide more tailored suggestions.

Her sister is willing to take care of her at (mother's) home.

Hmm. Is she indeed.

Does the sister have any clear idea of what's involved?
Do you agree that the sister is up to the job?

Let's say it's a very much younger and highly domesticated sister, with a background in nursing and a bad case of empty nest syndrome; fit and well; financially secure; looking more for meaningful occupation for her retirement years than anything else. In that case, great! Google "family caregiver contract in [your area]" and you should be able to find plenty of templates with instructions.

How old is your mother's sister?

If sister is close to Moms age, I wouldn't even consider it. With you Moms physical problems I would say there is some measure of help needed such as helping her to the bathroom, getting into the tub to bathe. At 72 I would not want to be doing this let alone in my 80s or 90s.

Getting paid? Can Mom afford it? If so a contract will need to be made up and I would want a lawyer involved. Medicaid, if ever needed, may question the payments otherwise.

In some States Medicaid will pay a family member but it won't be much and Mom has to be close to poverty level to get the help. Then as said, there is the VA. If Dad served during wartime, then there is Aid and Attendance. Your County VA dept will be able to help u with that.

Otherwise, there are really no programs that pay family to care for family. Office of Aging and Medicaid may provide aides.
