I have heard about Medsked a sleeve that fits on the medicine bottle, and helps one in remembering the dose that one have taken. Is this kind of product helpful in remembering the dose taken?

From what I've seen after googling the product it doesn't look appropriate for someone with dementia, it is more appropriate for the average person with good cognitive function who takes a lot of meds and needs a reminder. Even in that case I don't think the tiny tabs and font are terribly friendly to the elderly and others with vision difficulties or poor coordination.

I think a med planner box would be better. You can get them that have daily boxes for a whole week. This sleeve is not good for someone with Dementia, as said.

If the person u are caring for can't remember if they have taken a med or not, then their medication should be monitored by someone else.
