Is mom still in rehab? If so, there is usually a house doctor at the facility.

Does mom like the doc? What medical conditions does mom have? How old is mom? What is the doc's specialty? Do you have mom's POA?

Can you provide more info? Is Mom able to make decisions herself? Is she in a facility? Typically, you just make an appointment with a new physician, but it seems like more is going on here.
