Some states will pay a family caregiver to provide care if the person on Medicaid qualifies for it. In California the program is called In-Home Supportive Services. Apparently the pay rate is low.

Check with local Medicaid. There are programs called Adult Foster Care Program and it pays the caregiver a decent amount of money that is tax free to care for their loved one. There are some restrictions and what I understand spouses are excluded in the program.
Good luck.
