I have a sibling that just had a stroke, she has no idea who anyone is or where she is at. Someone needs to make decisions for her, she can no longer live on her own and we need to get her into a nursing home.

Too late for a POA, she cannot sign one. Look through her papers to see if she has designated someone. If nothing is already written, your only recourse is to pursue Guardian status by petitioning the Surrogate's Court.
