One suggestion is to contact your local agency on Aging. Go to the website I linked below.... click on your State... now click on your City/County. Said group could direct you.
I believe that any of the subsidized senior housing can be applied for by low income disabled folks. in my state, NH, there are a number of these in a variety of locations. (I don't know any with pools) but you will have to be specific about "accessible needs" when looking as we found only a few were truly accessible. My mom was in one for about 10 years. The last 2 years she used a walker and had to go up 4 steps using one railing to get in and out of her apt. There were only 2 Handicapped units (no steps etc.) in the complex of about 50 apts. We eventually moved her out because she needed more than she could get there.
