Many major insurance agencies offer this product. Be very very careful when choosing a policy because there can be all kinds of snags and restrictions. It is quite an expensive kind of insurance and usually only covers a set number of years. Certain diseases will make you inelligeable for coverage. The older you are the more expensive the premiums will be.
I have not done the math but if you are still quite young it may be a better financial options to set aside the money you would pay in premiums for care in your later years.
If you have enough money set aside for six months nursing home care most facilities will accept you as a patient and if you then need medicaid they are not allowed to transfer you to an inferior facility. When you are admitted with only medicaid you usually have to accept the first bed that becomes available in any local facility that accepts medicaid.
Lots of things you don't want to plan for when you are young but like everything else good planning gets good results.
