I live in CA. He has 4th stage lung cancer, and less than 6 months to live. No doctor has referred him to a social worker, he seems to have fallen through a crack in the system. His Oncologist just told him he has less than 6 month's to live, we both think he has MUCH less. He rents a room in a boarding house. He is legally disabled, and on medicare. He turns 60 on NOV 5th. He needs care outside of his rental room. He needs a case manager to co-ordinate care. I do not know how to get him either.
He is on heavy pain meds, and the hospital recently installed a pain pump under his skin. He has lymp-node and spinal involvement. He is NOT on antidepressants or anti-anxiety meds, but I think he should be.
I am his POA but he doesn't seem to be able to get HIPPA forms signed for me. I don't know how to get this done, I have talked to many of his doctors and asked them to put a form in his files to remind them on his next visit. (in case he forgets).

Pardon me while I read into this. He doesn't want to talk to a Social Worker and he does not want to get the HIPAA 'Health Care Proxy" to you. Hospice can certainly talk to him at a boarding house, and they manage the care, but obviously he won't talk to them either. About all you can do is make the boarding house manager aware of how to contact you when he collapses.

Download a HIPAA and bring it to him. Make copies.
