Someone I can trust. Someone who can make sure they are safe. They require 24 hour care. So it would be in shifts. That they take their meds. Go to their appts & keep them company.

The cost for in -home care 24/7 will end up exceeding the cost between a nurcing home or assisted living homes. All elderly people want to stay living in their homes who wouldn't. But, it comes to a point to where you need to decide what is best for them both and you. Can you trust the in-home care givers to be there everyday 24/7. Can you trust them. Regardless your'll be going to their home checking on them to see if they are being cared for correctly. Many worries on your table.
You sure can't trust family.You never mix business with family.
If your Parents can't take their meds on their own and require 24/7 care?
They don't need to be at home.They need a nurcing home or assisted living.
Getting you or them to agree to a nurcing home or assisted living home is another task to jump over.Are you sure in home care is the best for them and you?

Dev; If your parents require round the clock care and they have the funds to pay for it, you should call up the discharge department of the best hospital in your area (preferably a teaching hospital, one that has a medical school/university affiliated with it) and ask which home health care agencies they refer to. Call the Visiting Nurse association in your area for the same information. You need an agency because you want licenced/bonded caregivers; also because if one of the caregivers gets ill, can't come, you want the agency to arrange for a replacement. All that being said, it is MUCH MUCH better for the health of most elders to be someplace other than home as they age. Assisted Living, if they are well enough to be able to benefit from that level of care, provides better meals, more socialization and activities than they will ever get at home. I was amazed at how much healthier my mom was after 2 years of IL/AL, both mentally and physically. Call the care agencies, but also start looking at assisted living places. Most offer tours for prospective elders with a free lunch; that could be a nice outing for your folks.
