The hospice that the family member is associated with wlill gladly help with this transition. They can connect you with hospice's in the area in which the patient is moving to. A carefully thought out plan needs to be coordinated with making sure that the patient has enough medication and all equipment needs for the trip and depending on how far the patient is traveling some hospices preferr to have the patient discharged from their services and readmitted to the new hospice as opposed to just a transfer process.
