I think my client need 24hrs care,I have been working same Client 76 w/f whom had Stroke 2 years ago. She is getting much better now then first time when I met her, but a year ago she experience to another mild stroke.....since then I started work 24hrs for 4days a week, other 3days are another caregiver. I do pretty much everything most home care service that agency provide. but I do meds management... prepare for every week.

My concern is her daughter and my client think, they do not need 24hrs care anymore....just needs 12hrs, it's mean she will be by herself 8pm to 8am. She is incontinent, and hate to take a bath(only once a week) I help and keep her smell NICE!!

I have been doing this type work over 20years, I could tell her mobility are still weak,I'm afraid when she wants go to bathroom by herself or sometime she is forgetful. I understand patients right. Her daughter thinks she should stay by herself that make her more independent....I told her daughter to contact Mom's Dr or PT.... Please someone tell me what do you think about this matter?

I work with elderly for years now. Every day is different day as well the night time. If she will sleep OK one or two nights doesn't mean she will be the same every night. They get mixed up sometimes and think, like they can go to the bathroom themselves or start to get dress and they trip and fall, break bones and they will end up in the NH or at home with 24/7 care. You don't want to take chance. You did your part now it's up to them.

I'm sorry to hear this, people are often in denial until something drastic happens. If you don't get any response from the daughter, after a week of 12 hours of care, place a call to your local area agency on aging and ask for a welfare check. They can step in as an unbiased opinion.

You were right on target by stating your concerns and suggestions to the daughter. Another suggestion is to have the daughter call in a geriatric care manager for an unbiased assessment. Another thing I would try to suggest to the daughter if she feels so strongly about reducing the hours- ask her if they can start slowly like one or two nights first to see how it goes. Going from 24 hour to 12 hours is a big change and maybe if they go a few nights without care they may feel differently. A lot of people think that nothing bad will happen at night- and if they don't get out of bed that they'll be ok. Don't feel bad, at least you tried.
