I'm trying to determine when and where to move Mom and Dad. Dad will die within 12 months. Mom will not be able to live alone.

is there an assisted living situation now that will give Dad day to day care (with emergency assistance as needed) and provide Mom a "normal" day to day life within the facility. Mom is active, social and mobile (needs to stop driving soon). Dad wants to sit in his chair and sleep. he won't eat unless reminded.

Can they still sleep together to the end? or will they get used to sleeping apart? What kind of facility do we need?

Find Care & Housing
Even multi-level care facilities would likely separate your parents. Under the circumstances, I would keep your parents in their own home until your dad dies. For now, call in hospice and also hire caregivers for your mother. Towards the end, your father will be in a hospital bed. Once your father has passed on, move your mother into a care facility - sounds like she needs Assisted Living. Make sure that it has multi-levels of care so that she will get everything she needs without being moved from one place to another.

Look for facilities that offer "aging in place" which means they care for people through a progression of health from independent senior to moderate assistance to total care. Hospice services can be provided in the home so moving isn't necessarily needed. Medicare covers hospice 100%. If one or both parents are veterans they're likely eligible for a suite of at-home services.

Start the conversation with their MD, since any recommendations for placement or assistance start with the MD ordering in home nurses or aides. At the very end, he will likely be in a hospital bed provided by Hospice. If you can keep them together in their home for now, excellent. But I would start looking at ALF's for Mom to move to after he passes. I doubt they could be together in an ALF unless you can find one with a Hospice wing.
