My Dad has nightmares, falls, and is really weak in the middle of the night. Any advice?

Can you say more about his nightmares? Many sleep disorders are highly treatable.
Does Dad fall during the day, too, or only at night?
Is Dad on any drugs?

It sounds like it may be time to place him in a nursing home or your mom may join him. Check into options before it becomes an emergency placement.
