My mom suffered a stroke in August 2016. Since then she has been in a nursing home. I'm unable to find any photo ID and her SSI card. We live in PA. I spoke with the social security office and I have to download a SS5 form and have my mom sign it. What kind of ID do they accept? From what I understand they don't accept copies and want originals. Do I go to the nearest SSI office?

Birth certificate and marriage license were what I found first - at the county registrar's office where these events occurred. As the child of the person, you are allowed to buy copies (in most places). This gave me the ability to have those alternate documents for the form to get a replacement SS card, BTW, SS offices don't recognize POA documents. Have her X the form if that is all she can do, with 2 witnesses, 1 of whom is not related.

To replace license, you will need those documents, and others depending on your state. Contact your DMV. I was able to take mthr down to the office with her bc and ml along with what her ssn # was without the card. They were more than glad to issue a new ID card and whispered to me, is she turning in her license. Of course I said yes! They were so happy. Mthr had hypoxia so she was really out of it, and it was a smooth operation.
