Your boyfriend's father would need to write up a legal agreement between the two of you that he will pay you so much per month for your services which he needs to state in detail. I would imagine that this document might need to be notarized, but I'm not sure.

It is very kind of you to consider doing this You might want to search this sight about caring for people with alzheimer's/dementia. You are going to need some sort of an escape clause because it will reach the point where his need for 24/7 care will exceed your ability to provide. You will need to set some boundaries so that you take care of yourself and your relationship with your boyfriend which must be heading in a long term direction since you are interested in becoming a paid caregiver for his dad. We wise and be careful.
