I work for a man with dementia. I work 24/7 and have not had a day off in two months. The family will not cover a shift for me. I am wore out. My client is very angry and aggressive. If I tell the family how I am feeling they will most likely fire me. My health is beginning to suffer due to the stress. I am paid very well.

I'd say, since you're paid "very well" and you need a break, that you should get permission to hire a respite care giver. I've done that for live-in care givers, and the family was fine with it.
