Mom had a stroke – was in her primary hospital for 10 days, then moved to SNF. At SNF she contracted sepsis and pneumonia and went to a DIFFERENT hospital to ICUs, then to a reg room. Now she is in a NEW SNF. Had to take off work for 3 weeks due to her ever-changing health status, not to mention finding the new SNF, daily visits, calls with doctors, nurses etc, paying her bills, getting her a new phone and new glasses (she lost hers at first SNF). Hospital she was at refused to sign FMLA and faxed her papers to her primary care doc. Primary care doc filled out the paperwork AS IF her condition was what it was 2 months ago when she FIRST went to hospital – not to mention she marked that my mom was "pregnant". Which is NOT the case. Any suggestions on this type of thing? TY!

Does it have to be the old PCP? If he's in a SNF, wouldn't the doctor there have access to a more complete record of recent health events?

I made an appointment with a doc to fill out a form needed for the guardianship process. Nice doctor, had never seen him before or since; I think he was the doctor on staff at the SNF. But he willingly went through it (with me offering lots of commendation for dealing with ridiculous questions like "How long will the patient have this condition?" Doctor: "It's DEMENTIA.")

After 20 minutes the form was done, I thanked him for his forbearance and got the completed form to the attorney As Soon.

Contact the doctor's office. Tell them that he/she filled out the paperwork wrong. Tell them you will bring in the paperwork to be completed. Give them a date/time you will pick it up. Be sure to review the paperwork before leaving the office to be sure it is done correctly this time. Be sure to make a copy of the paperwork for your records.

Duff I don;t know how this will work for you but alot of us just fill out the paperwork ourselves and the Dr signs it,, my gal was always glad I did it myself, and I took it along to one of MY apts, so I never had to pay that extra "fee".. She would review it at my apt and sign it and off it went! Plus I knew more about moms conditions that would effect my job then she did for sure!
