What should I be asking my mother's nurses about her care?

What should I be asking nurses/PAs about my Mom's care. Recently, we found that Mom's back was not looking right. It is covered in moles. Some of them looked like it had developed dark indented moles (or something else). Also, when we visited Mom was wet up to her navel. They said after eating, Mom will pee and they don't know about it. They said they will take notice on a regular basis.
