I have Medicare Advantage that pays for 100 days in a skilled nursing facility. I was told that they do file Medicare Advantage and they do it as a courtesy Somehow the girl did not get preapproval for my husband and the insurance is denying the 100 days because of her error.

Did the facility biller contact you while your husband was in there, any time during that 100 days? Hard to imagine you did not find this out until after the 100 days was up. ... Always check with nursing home billers to stay out of hot water with them.

Appeal the denial with support of the facility. They should provide you with documentation of their error.
