Who is my contact when my family member is in subacute rehab?

Do you have POA? If not, no one can give you info if not on HIPPA paperwork.

I have never had any problems getting info. For me those Care meetings are a joke. Its them telling you what they are doing and no time for your questions because mine were only 15 min. The only thing was, I found out my Mom had not been given her Thyroid medicine because a Dr. at the Hospital felt since her labs came back normal, she didn't need it. They were normal because she was on medication. No call to her endocrinologist who was treating her.

I have never had any problem getting information from the RNs at the Nurses desk. If they aren't obliging, then the DON (Director of Nursing) would be next. If she isn't forthcoming you keep going up the ladder. If you are POA take a copy and have it put in Moms files. Make sure its noted on her chart that a POA is in effect. In the system too.

If your Mom has no Dementia and is competent to make decisions, staff cannot talk to you about Mom medically. You will need to get the info from Mom. Have her put u on her HIPPA paperwork as someone staff can talk to.

My mother was in the hospital (and rehab) during the big Covid surge last winter. The only people I could reliably count on to get any information were the case managers. A doctor called me once during her entire 14-day ordeal, and I got hold of a nurse once, but since the case mgrs are tied to their desks, they were actually reachable. They couldn't really answer any questions since they'd never seen Mom, but at least I could get the general info on her current situation.

Hi - After moms stroke she went to subacute rehab as a local skilled nursing facility. I met with the case manager (I was moms POA as well as healthcare proxy) and I met with her first. The director came by the room as well as a few others (drs etc )but we were just at step one in those first few days and I was clueless.

Was your mom just transferred there? For what and do you live close by? Do you have POA or health care proxy/surrogate?

Start with meeting the case worker - she will set up the Care meeting (it should be in the first few days) and will include a few different people in that meeting - from the director of nursing - some or all of the therapist involved in moms rehabilitation - head nurse - director on site - even dietician (as my mom was on a peg feed tube in the beginning).

You can put together all your questions for that meeting.
A rehabilitation schedule and terms/outlook - what medications she is on and why - drs names overseeing her care etc.

It really depends on what your mom is in for - that will help others give you advice on who to meet and what to ask. Maybe a little more information will allow others to give you more specific questions to bring to your Care Meeting. 🦋
