Where to find a transport service to go from Sellersburg to New Salisbury?

My Mom is in Maple Manor Christian Home in Sellersburg In. She has an Eye appointment with Dr. Masden at Insight Eye Care In New Salisbury In. She is in a wheel chair and can't walk or stand on her own.

The home should provide wheelchair transport. Is there some reason they won't? Are you crossing a county line?
