Has her Medicare paid rehab ended yet? if not, please please pls do whatever you can to have her going and doing her best in rehab. If that means you go an do a little cheerleading routine with her daily, do it if at all possible. MediCARE can pay for a portion of rehab up to 100 days but she needs to be progressing in her rehab plan. Try to keep her in “progressing” status and not hit a plateau.
Just when she can apply for LTC Medicaid dependent on how her state runs the program. Some allow for it to go ahead of thier final spend down & impoverishment, while others have it that they can only apply after becoming impoverished for LTC Medicaid and showing that in their bank statement. Often NH that have Medicaid beds, have the application system such that the POA does the application at the NH & admissions kinda helps them fill it out and then the NH submits the elders application along with the NH room&board bill to the caseworker assigned to that NH / zip code or region.
If where she is now is also a NH and she is planning on staying there to become a LTC resident after her rehabilitation patient time is over with, the NH admissions person should have a list of all the financial / asset & health care related items that will need to accompany her LTC Medicaid application. It could be 1 or 2 pages of a list….. usually includes 5 years of financial documentation, so maybe last 6 months of bank statements and and then every six months back to 2017; her “current “awards letters”…. these are the annual statements from SSA, pensions, annuities that state to the penny what she will be paid in 2022 (they get mailed out Nov/Dec 2021); an ID, birth certificate or naturalization papers; health and life insurance policy; perhaps her will as well; and title or current tax assessor statement on any real property (house, land, auto) even if she sold these in the recent past. Plus whatever other items this particular NH want to see.
A NH can review all the documents to see if anything looks like it may be a “gifting” issue for Medicaid. If so a NH may not accept them as a Medicaid Pending resident. You need to clearly ask that they are there as Medicaid Pending otherwise someone will have to personally sign a contract to be financially responsible for the elder.
