I've had 6 heart attacks, and my lungs are at 25% - 30% working.
I fall often, leave the stove on and forget. Can't do much by my self.

Not sure about your financial situation, but you may want to start by contacting social services at your county's Dept of Human Services. Request they come in and perform a needs assessment. They will probably ask why you are requesting an assessment and you can give them your concerns at that time. Make sure your doctor performs a baseline cognitive exam on you so that this too is in your medical records. If you have no one who you'd trust to be your PoA, then you can discuss guardianship options with social services. I hope you find all the help you need and want.

I'm sorry to hear you are having problems--BUT, God bless you for NOTICING and not ignoring the changes, as so many people do, just wreaking havoc on all those around them.

I would THINK that a simply dr checkup and asking for some kind of form that gives you the status of an adult in need of help would be best. Chances are any from you download is going to require a dr's sign off anyway.

And there is a chance that your drs office will have information on placces you can investigate on your own.

Actually, to hire a private care agency, I do not think you need a dr's say so--you simply walk in (or get someone to go with you, for moral support) and sign up!
