What kind of form do I need to show proof I am my parents caregiver?

My Moms doctor provided a letter for me saying I was Moms sole care giver.

I would think that an official diagnosis that mom has a condition that requires a caregiver would be the 1st step.

If it's for jury duty or some sort of official purpose, a letter from her doctor is usually what is needed.
