If by insurance you're thinking of the liability insurance you'd have to carry if you hired someone directly, w/o going through an agency, your insurance agent is the best to provide this information.

Rates can vary; I called over a dozen private duty agencies to get comparisons and try to find one I liked. In the SE Michigan area, they ranged from $19/hour to $26 an hour. The $19/hour one was at the low level in service at well; the owner was annoyed when I indicated I didn't want any smokers as caregivers.

You really need to contact homecare services where you live. This forum is not privy to information by state unless one of our members live in the same state.

Call your Office of Aging and ask what services they provide or info they have on private services. If patient is low income, Medicaid maybe able to help. If by insurance you mean health insurance, Medicare does not pay for in home care. You could try the patients supplimental insurance but I dought any help there.

I talked to an agency yesterday called A Place for Mom that gave me some really helpful info on pricing for both home care and AL services in my area. The lady I talked to even set up a tour of an AL facility for us for this weekend.

I think they are a nationwide agency, and if you call the main number they will transfer you to an agent in your area that can help you find local pricing according to your needs. They were more helpful than most of my Internet searches, as most of the places here don't list their pricing on their site.
