GCM A Geriatric Care Manager can help you if you feel you cannot manage the process yourself.
Don't feel like you are alone if you cannot do the paperwork. There are people who help with the process, but I am not sure of the titles. Some are social workers, care coordinators, paralegals, etc.
If it is just the technology, computer use, copying, faxing the documents, a trustworthy tech person (in the family or hired) can help you with that.
Are you DPOA. Is Mom competent to sign or does she suffer from a Dementia.
Are you just transferring her to LTC. If Medical records are needed, then all Mom needs to do (or u if DPOA) is sign a form that the LTC can request medical records from certain doctors. The doctors may want a form signed that its OK for them to send on medical records to the LTC.
If you are talking about a Medicaid application, I would be a little leery on allowing the facility to do it unless very experienced. I would be on top of them the whole way down to knowing who the Medicaid caseworker is they are working with. In my State you only have 90days to complete the process of spending down assets, getting Medicaid all info needed, and finding a place for the person applying for Medicaid. The only person able to give the LTC this info is the recipient or you. Medicaid gives a list of what they need to complete an application.

Are you referring to necessary information as in medical records to show your mom is medically at need to file for Medicaid?
You posted under paying for care so that’s an assumption on my part.
A bit more information from you as to your mom’s situation would help with answers.
