The Participating Communities may contact you directly regarding their services. APFM does not endorse or recommend any provider. It is your sole responsibility to select the appropriate care for yourself or your loved one. We work with both you and the Participating Communities in your search. We do not permit our Advisors to have an ownership interest in Participating Communities.
II. How We Are Paid. We do not charge you any fee – we are paid by the Participating Communities. Some Participating Communities pay us a percentage of the first month's standard rate for the rent and care services you select. We invoice these fees after the senior moves in.
III. When We Tour. APFM tours certain Participating Communities in Washington (typically more in metropolitan areas than in rural areas.) During the 12 month period prior to December 31, 2017, we toured 86.2% of Participating Communities with capacity for 20 or more residents.
IV. No Obligation or Commitment. You have no obligation to use or to continue to use our services. Because you pay no fee to us, you will never need to ask for a refund.
V. Complaints. Please contact our Family Feedback Line at (866) 584-7340 or ConsumerFeedback@aplaceformom.com to report any complaint. Consumers have many avenues to address a dispute with any referral service company, including the right to file a complaint with the Attorney General's office at: Consumer Protection Division, 800 5th Avenue, Ste. 2000, Seattle, 98104 or 800-551-4636.

Mom's been bedbound for years can't walk without falling been hospitalized 44 times in 6 years for other comorbities I want to suggest palliative or hospice, no hope left I know she has reached her limit, no one else agrees
Both UTI and bowel impaction are treatable. In some cases the bowel impaction may require surgery and the question is would she survive surgery.
You do not go into what other "comorbidities" she has. Is mom cognizant? If so what does she want.? You (I am going to assume you are her POA for health) can request a Hospice evaluation at any time. If you are not POA and she does have one then you need to follow those guidelines. Who are the others when you say "no one else agrees" ? If it is the doctor then you can by pass the doctor and contact a Hospice of your choice and let them evaluate mom. You could also request Palliative Care. That will not change her doctor, it will add another layer of care. The great thing with Palliative if they think she is more suitable for Hospice they will let you know.
lastly...If mom is cognizant and she has "reached her limit" she can chose to end her life. There are States where Medical Aid In Dying is legal. You can look into that. It is complicated and not an easy process. Or mom, again if she is cognizant can chose to end her life by VSED Any mentally capable person can chose VSED (Voluntarily Stopping Eating and Drinking) This process would take anywhere between 7 and 21 days but 10 to 14 is average.

UTIs and bowel impactions are not terminal medical conditions, and neither is immobility -- what are her other comorbidities?
No one can blame her for feeling miserable and hopeless. Does she have an active MPoA? Can someone find a different primary doctor? I think the hospital self-referral sounds like a reasonable option.
Also, if she's bed bound why did she have a hip replacement? She won't do PT if she's in pain so her surgeon or post-op care team needs to address this -- but it won't help with her constipation. For the time being she needs way more management than she's getting so she can get out of this mire. It can't be left to the various involved medical personnel... someone has to orchestrate and strategize for your Mom, at least until she gets her current issues treated or under control.
If your Mom doesn't currently have a DPoA it may be time for her to do this asap before she can't.
We self referred twice to hospice. You pick up the phone and call them and an RN came out and talked to Mom and us. The RN pulled Mom's medical records from Mom's PCP. Both times Mom was admitted to hospice.
It doesn't matter if "no one else agrees" as this is a decision that your mom can make herself if she's in agreement that there's "no hope left." You can call the hospice agency of yours and your moms choice and have them come out to do an assessment to see if in fact she qualifies for their help, and take things from there.
If you mean by no one the facility staff, thats not their choice. If Mom is competent, she can request hospice. If you are her POA, you can request hospice. You can call them in for a consultation. They may have to get permission from the facility to come onto the property. But I would think you have a right to consult with them.
