Any suggestions: My 58 year young brother has been in a nursing home for 4 months. Unfortunately he has dementia, caused by years of drinking and a numerous head injury. He has been in and out of hospitals, rehabs and has a history of behaviors. Alot of facilities have declined his admission due to his history. He is now in a nursing home that is 1 1/2 hours away from me and has regressed so rapidly, (confined to a wheelchair, needs assistance with eating, hygiene, feeding, etc). I am able to visit him only once a week. The times I have been there I have had concerns about his care, ( hygience, social interaction, belongings disappearing, fallen twice sustaining injuries).. One occasion he was in a geriatric chair and his clothes were saturated with urine. (Obviously he must of been like that for hours)!!!!!! I was quite concerned and a case plan meeting was requested. I have been asking repeatedly for him to be moved to a closer facility so I can visit him more frequently. It is so important for his well being as he needs family support and I can monitor his care. His social worker supposedly has been checking on facility, however, I have called a few and they said, no records of her calling!!!!!!! I have asked for her help and suggestions as to how I can help with this process and she has no answer. She never returns phone calls and previously I went to her director to let him know what was going on. That worked for a few weeks.... I just need some suggestions as to what I can do to get my brother moved closer...I am his guardian and just want the best for him. My concern is, if the social worker cannot help with this request who can I turn too. I am considering filing a complaint and hopefully this will get some answers....

To move my husband's parents, we visited nursing homes in our area and the ones we liked were sent copies of medical records for them to review for admission. We were up front up when we discussed the ILs and the care they needed. Once they were accepted we moved them. Since you are your brother's guardian it seems it would be similar for you. Wouldn't you rather review and choose the next facility for yourself? The social worker at the new facility should handle whatever details you are used to the other social worker handling.

You might try going to the ombudsman for that nursing home. This is your (or your brother's) representative. If you go to www.ltcombudsman.org and type in the Zip code of the nursing home where your brother is, you should find your contact. You can tell this person about the conditions where your brother is, and also ask how you could go about transferring him. These people are trained to help people with long-term care problems.
