To get guardianship, if you only have one primary doctor that administers dementia medicine, what second source would they accept?

You would need one other doctor like a gerontologist or a neurologist to diagnose her as incompetent to manage her affairs in a business like manner. Has the first doctor already diagnosed her a such?
