Does anyone know about
The Nursing Home Transition and Diversion (NHTD) Medicaid Waiver Program. How long does it take to get approved?
If I am not getting any response from my service coordinator, who do I
reach out to complain -for five months and still waiting to bring mom home from the nursing home. I feel hostage to the system. I am exhausted with all these delays, and mom is getting worse staying in the nursing home.

Did your elder have a needs assessment done? If so, what is it showing for her care plan? If it’s over 30 hrs and in any way complex care, they may not be eligible for a back to home transition program.

the program (Texas has something like this for reducing NH beds) kinda is best for NH residents whose care is now really custodial but they entered a NH for a rehab stay after a hospitalization…. Then got better and not actually needing skilled nursing care. If it’s as you wrote that your mom is actually getting worse over time, her assessment may show she needs in a facility care. That there is no safe discharge from the NH for her

Thank you for your response - I am not the open who fills out the application
a service coordinator does - Assessment is a must - for the NHTD to decide how many hours she needs - I thought the program is for dementia patients. They are aware the patient will never get better - mom has been getting worse since she entered the nursing home for lack of service - I wish I were not around to see this day- it is heartbreak

Medicaid is run independently by each state so others from NY will have to answer your question.

Also, I just heard on the news last night that NY (city?) social services is being overwhelmed by an influx of migrants and there's only so many public employees -- they just can't keep up.
