My dad has vascular dementia and he has had hallucinations for over three years.

Hallucination are not unusual. If they scare her there are meds. If not then just go along with it. Dual conversations too r normal.

It could mean she has a UTI. Hallucinations are a common symptom. You could do an AZO test at home for starters, but a culture done at the Dr or urgent care will give you a definite answer.
