Patient was initially given level 3 on 8 level scale. I requested but did not receive services they expected he would need. This is a new program for the facility but there is a considerable price differential in the different levels. I have POA for patient and need documentation for my records.
Ask administrator - be specific telling him/her what you want, in writing.
Read your contract / updated information re level scale/payments/charges.
Document everything.
Know you can contact their licensing board with any concerns and Better Business Bureau (if they are still in business) ... and the Attorney General (although nowadays... I don't know about them, either).

So for example level 1 might include.
Some assistance with daily tasks, light housekeeping
Level 2 might include
Bathing, dressing as well as what in included in level 1
Level 3
More help with ADL's, toileting, medications
Level 4
More supervision, more help with medications, more help with toileting possibly help with eating and drinking.
But each facility may have different breakdowns of what is included with each tier of care.
You should have been provided with a outline of how your particular facility scales the care.
And YES I think you should get a breakdown of what care was provided.
I do not think you will get an itemized breakdown though. (difficult to say how many times a staff person had to help XXX to the bathroom, or how many times they had to be helped with lunch or dinner.
I do think that if there is a change in the level of care that is provided there should be a Care Plan Meeting to discuss the decline of your loved one.
