My mother lives with me and I need to get POA & Guardianship, can I just get the paper work and do it my self?

You can get the proper forms for DPOA and Medical Power of Attorney (also called Medical Proxy, and usually included in a healthcare advance directive (living will) and fill them out yourself. Make sure that you follow any requirements of your state, such as having signatures notarized and filing the documents with the state, etc. (These vary by state).

The guardianship process must go through the court system. It is not really a do-it-yourself activity. But I really don't think you mean guardianship. As ElaineRenoire suggests, look up what is involved in guardianship in your state and under what circumstances you would really need it.

Guardianship is a drastic measure designed to be a matter of last resort after every other stone has been overturned. I would suggest before you go to that extreme, you look into doing a durable power of attorney. Some states have these forms on-line. Read up about guardianship abuse before you go any further.

You can get help from a social worker. They'll have the right paperwork and can help you fill out the forms. If you can afford one, get an attorney to help you. They'll do a better job than a social worker. Also, if you mom's of sound mind, be sure she understands what appointing you as he dpoa/guardian means beforehand. Both of you need to be fully aware of what this means and how your lives will change.
