She can walk with an walking aid, normally has a cheery disposition but can get agitated if there is too much activity around her.

Rocky, that is a kind suggestion. And if it were a calm, quiet weekend it might be relaxing for her. But it could very well backfire, too. She may get agitated over the memories the place evokes, or that she can't remember it, or that she doesn't want to return to the NH. I'd be inclined to try a shorter outing, as norest suggestions. If you take her for a meal, avoid peak meal time and try for a quiet setting. Or take her to a flower garden, conservatory, art museum, or someplace without constant activity and that suits her interests. See how she does on these short outings first.

Agitation is a response common for people who cannot process or handle activity around them. It is confusing for them. It seems taking her away from her structured environment may also cause some agitation. Not sure of her conditions, is she able to go for lunch? You just need a doctors order to do so. Bringing her home for just a weekend may be a huge undertaking.... and upsetting for her.
