Your Mom would need to apply to & qualify for however NH Medicaid is administered in SC. She would need to find a NH that will accept her at the level of care indicated in her last care plan at the old VA NH plus as an "Medicaid Pending" & out-of-state transfer. Whether this is hard or easy will be very much dependent on SC medicaid rules. I would imagine that the SC NH will require someone who is a legal resident of SC to sign off on a payment contract to be financially responsible for payment to the NH for moms stay if Medicaid is declined or takes over a certain # of weeks to process.
Medicaid is a joint federal & state entitlement but uniquely managed by each state. Medicare is totally federal & it can be used if a facility takes the Medicare plan you are on.
Moving from 1 NH medicaid bed to another can be done but will require work on your part to get documentation done and a willingness on the part of the new NH to deal with transfers plus the extra issues posed from coming out of state.
Medicare does not pay for nursing care, Medicaid does. Since Medicaid varies from State to State I would call your local Medicaid Dept (in my case, Social Service) and ask if they have any literature.
