I’m looking for a free testing resource. Paper printouts, online Q&A or other options are welcome. A quick Google search led me to many sites that require a sign up as they gather emails for sales opportunities. My dear brother is trying to assess his in laws Dementia situation. 80 yr old FIL has no medical insurance and his immigrant status will not qualify him for any benefits in Texas so my brother is on his own - just trying to understand where FIL is in the Dementia cycle.

This test is designed to be taken at home and then discussed with a doctor.
You can download the SAGE test directly from the Ohio State University Wexner Medical Center. They provide the test itself, scoring instructions, and additional information.
