It's been a while since I've been on this forum. 90 year father is currently in hospital with pneumonia and obstructed bowel. He is getting better but very concerned with him returning to his home where he lives alone. I have lined up care giving for him the past two years which has been working. But it's not enough. When no one is there, he falls and does not eat properly. Is this the best time to make that transition from hospital to a care facility? I'm afraid if he gets home, he will continue to decline and end up hospitalized again. I should add that he can be combative and very disagreeable. He has slight dementia, but in otherwise good health. Other then his current medical issues. Thank you all for any input or ideas that I have not yet thought of for him.

He is not in good health any longer. He got hit as an elder with 2 life threatening events from which when he finishes rehab may not bring him back to 100% baseline. Before discharge, try to line up a family meeting to see if he will reach his goals. The key question will be a safe discharge and if he can still live alone for several hours without care.

Its not safe for him to be at home anymore, certainly not with dementia. Get him diagnosed at the hospital with a MoCA or SLUMS test that's scored on a scale from 1-30. If he scores 18-25 = mild cognitive impairment, – 10-17= moderate cognitive impairment and – less than 10= severe cognitive impairment. As POA, you can place him in Memory Care Assisted Living w/o his agreement if he scores poorly on the test and gets a dx of dementia. Get him evaluated for agitation as well and medicated accordingly. He's likely further along than you realize.

Are you POA? Because if not you cannot make this decision for him. And quite honestly, if he is not legally incompetent under the law you still cannot. If you discuss with him and he is amendable to this move you are actually well past the time when it's appropriate if he will agree to it, and one of the easier way to accomplish it is with the help of the hospital Social Workers or rehab social workers.

Now you mention he is combative and unreasonable.
There's the rub.
He will not be kept at a facility if this is the case, so it is time to discuss medications that may help. When you look at the admissions packets for ALF/MC they will enumerate the circumstances under which a resident will be requested to leave. And they are exactly those circumstances you mention.

Yes, I am POA, so that hurdle is done. I have tried to keep him at home as long as possible, but I just don't think it's safe for him anymore.
