Local senior citizen center or senior service. My parents small-mod size town in Texas provides through a second party. You do have to make arrangements 1-2 days in advance. Charge is 2-3 dollars, each way. And they may need to wait 30 minutes or so. They have to be pretty with it, for it to work, best I can tell.

1. Aging and Senior Services, centers, agencies in your area.
2.Get referral from PCP to Social Services who will be able to assess your overall status/ needs and provide options in your area.
3. Register with "GOGO Grandparent" ....rides, transport, delivery of food etc etc

If the person has transport and drop off, are they of sound mind to get inside the dr's office or into a shopping center? Can they manage carrying shopping bags and understand what the dr says? If they are ok medically, mentally, and can ambulate on their own, all you really need is the transportation. In that case, use taxi or Uber/Lyft for the transport to and from. Your person can call for ride when they are ready to return home.

If there are medical issues, you might want to check to see if there is a medical transport company in your area to assist with the dr appts. Private transportation with aide to assist for shopping.

A Senior Center exists in Azle, TX, in Ash Creek Park. Jackie Joy is said to be the director of 817-444-0070. Address is 601 Southwest Parkway in Azle,TX They have nutritious lunches, activities, and some van trips. Frequently at Centers such as this, "someone" will know of resources for low cost transportation even if the Center itself does not provide transit for shopping and medical appointments. "Lunch" is served daily at 12 noon. If you are able to participate in a meal or even an activity or two, you may learn of transportation services offered by churches, the City of Azle, the County, etc. I hope that this interesting sounding Senior Center has ideas for transit that can meet your needs...I can't predict what they will know or will offer, but a Senior Center is a good place to begin your search.

Check with elder services or your Local senior center . I use LYFT But I am In a Big City .

Redcross helps with transportation. If you have Medicaid for heath or in home services, they too help with transportation.

Many large churches have Care ministries. Contact some nearest you and see if they can provide a volunteer driver. My church did this often, and for people who never attended our church, or any church.

Or, join Nextdoor.com and ask non-anonymous people who actually live in your community for help.
