Probably because she has dementia. What was she like before she got sick?
Since she is in a facility rather than living with you, I would think that her bullying couldn't be very effective. She surely can't make you do anything you choose not to do. You can simply walk away.
And that might be a good practice. If your visit turns nasty, say, "Gram, it seems you are a little upset right now (or angry or tired, etc.) I think I'll leave now and come back on Wednesday. I hope you'll be feeling better then."
