Maybe try looking at his bank statements if he'll let you. They have to have been charging him annually/quarterly/monthly. At least if you have a name to go off of and call with his identifying information you may be able to track down more policy information.

Kathleen, usually we have to put on our detective hat and start going through our parent's file draws, desks, bookcases, closets, and look for the long-term-care insurance policy.
