My mother just went to a nursing home in Kentucky after a hospital stay. The doctors believe she will nver be able to live on her own again. I live in another state and want to move her to the state where I live so she will be next to be. She has agreed and since she is in there for "short term" right now who do I contact to get the ball rolling and to get her settled before she becomes sicker and/or passes away.

Find Care & Housing
contact the social worker or discharge planner at the nursing home ..they should be able to handle all the paperwork.. make a list of all your questions..if you have poa for financial and health the process will be smoother. i hope you do...looks like you might have to take a trip to kentucky to pack up moms apt..unless you have relatives that can do that.. but make your list of questions and call the nursing home. good luck with everything

No, I guess it was more like how do we get her SSI transferred, what about bank accounts, do I contact the new nursing home and they make the arrangements or does the nursing home she is in now make the arrangements? What should I do about her apartment? What about her medical records, new doctors?

If you're wondering about the actual physical moving of her, then I would think it would depend on how well she can travel right? Have you gotten your ducks in a row with the new place you want her in? You know there is always the adult foster care option too, not just a nursing home. I guess I don't know what aspect you're asking about. Sorry.
