It is something to discuss with your family members if she is 85+ years old and or is in the late stages of dementia to choose not to treat this.

But you don’t give us any information to discuss.

Her dementia will get worse after anesthesia and it’s very possible she will not be able to recover from the surgery.

You should discuss all this with the doctor, and let the doctor know the issues he/she will be dealing with. They will best know how to medicate for comfort during this needed surgery.

How bad is the dementia?
Does she already have behavioral issues , paranoia etc ?
Hospital delerium may happen . There could be a permanent step down in cognition.

You don't give us much to go on, like why your mom is needing a hysterectomy, how old your mom is, and how long she's had dementia, so it's tough to answer your question.
But I will just say that any time a person with dementia has to have any kind of surgery, you can pretty much plan on them have their dementia symptoms get much worse from the anesthesia, which can last a short time or be permanent.
And if your mom has dementia, which already is a death sentence, I'm not sure allowing her to have any kind of surgery is in her best interest anyway. That's my thought anyway.
Perhaps if you share more info, it may be helpful.

What if she needs the hysterectomy because she has something like uterine cancer? Do you really want her to die of cancer?
