I pulled her credit report, they all show charge- off. I have POA. Do I need to be concerned about these debts?.

Even if the original creditor has charged off the debt they may sell what is owed to another party who will try to collect. Each state has a statute of limitations on collection.Some parties will even try to collect even after the statute of limitations has expired. That is called Zombie Debt. Do not, DO NOT contact these creditors and keep that statute running. If the money is there to pay the debts go ahead and pay them, but I'm guessing it isn't. Get a free consultation from an attorney about this, but some things should just be allowed to die. Do NOT talk to these people or acknowledge them and for crying out loud do NOT start making token payments. You would just be keeping the debt alive. Everything has a natural lifespan, even debts.

What do you mean by "charge- off"?

Even if your Mom has dementia she is still liable for any debts, if any debts do exist. Have the collection agency send you a copy of the debt so you can review it. If the debt doesn't show up on the credit report, it could be it wasn't record as the vendor is still trying to collect.

You being the Power of Attorney, you will not be liable for your Mother's debts.

charge off means that the debtor wrote the debt off.
