She was ill back in Feb. and March so we had to keep her at the facility since she lives alone, she couldn't go home the way she was. Now she seems like she's back to her old self and I was wondering how do I get her back to the rehab part of the nursing home ?
When Mom was in the NH she did get some PT. If you bring her home her PCP can order in home PT.
Getting her back to rehab? That would be between you and the Social Worker. If she has any of the initial 100 days and the diagnosis is the same, she may be allowed to finish out the 100 days. If she had already done her 20 that Medicare pays 100%, she will not be allowed them again. She or her suppliment will have to pay the 50% that Medicare doesn't pay.
There are probably a few different answers to this question depending on several unknown factors.
The type of insurance your mom has will be important as different policies have different benefits.

Has your mom been private pay while in LTC?
I would ask the doctor to order her a PT evaluation and see where she is on being able to do therapy and then discuss how to get it for her.
