Hi, my mom has Medicaid and is currently in skilled and rehab place. She had post surgery complications and was in hospital for month and skilled Long term hospital for two months. Now she is in skilled nursing and rehab place for 36 days. She only had therapy for 2 weeks, three times per week for 15 minutes!!! The care is horrible. A lot of nurses from agencies. She is on TPN line and has wound with fistula. She is stable, labs good. No complications. She declines physically; not able to move legs, stand up or walk. No PT is done for last 4 weeks. I am LPN and worked with such patients. I would like to take my mom home and asked doctor for discharge orders. I have home health agency that will help with TPN and wound but this place just making all the excuses. What would I do? I need order from doctor since HH agency will not take her without it. Help.

Can you go to the facility and work with her?

It would help you see if she really has the capacity to improve or if this is her new baseline.

I would be surprised if she got 24 hour care at home, so you should do everything you can to find out what is what and help her do PT at the facility while Medicaid is paying for her care.

She was having surgery to reconnect small intestine with colon; she had active ileostomy and inactive colonostomy. There was perforation in the place of connection. She is 74. I am POA. Fistula is between abdomen and outside (stitches in wound open out and fluids started to come through the wound area). She is NPO till it heals. Insurance doesn’t cover her PT anymore (she has Medicaid). She is stable but there is no one that does anything physical with her.

Ewa: Who is the POA for health care for your Mom? What did the doctor tell you when you asked to take your Mom home? Where is this fistula (between what and what)? How old is your Mother. What initially took her into hospital several months ago? Why is she unable to take nutrition? What is her prognosis according to the MD? When you have asked about why she is receiving no PT what were you told?
