I am POA for my uncle who is currently approved for New York State Long Term Mediciad and is residing in a nursing home. The annual renewal application is soon approaching. I am trying to be proactive and gather the banking documents, Social Security documents and his Pension statement.
Is there anything else that I will need in addition to his income documents and the reconcilation of deposits and withdrawls from his bank account?
Is this renewal process as cumbersome and lenghty as the initial approval process? I can't believe that it has been a year already!! Thanks for all your knowlege and suggestions in advance.

NOT as cumbersome as the initial application!
You will need to report and provide documentation for any changes in his assets since the initial approval.
Any personal property or money he has distributed, whether given away or sold; any personal property or money he has received in the last year, any significant changes in income or value of assets. Also, don't forget to include any changes in liabilities, such as loan balances or changes in his monthly cost of living.
We are in Arizona, and the renewal statement is already pre-filled with my income and my husband's SS income. I believe they get that information from the state or IRS income reporting for tax purposes. I am only asked if there are any changes, and if not, I don't need to do anything. Any reported changes require the relevant documents.

For my mom, the initial application was abt 130 pages. For the renewals abt 20 pages. Some items were the exact same ones submitted initially. It was last 4 mo of bank statements (every page that is indicated as to # of pages too), NH fund statement, last property tax bill, funeral & burial policy, the SSA & Pension award letters. For MIL in a different State it was done via a 2 page “yes/no” checklist plus last 3 months of bank statements and last SSA award letter (the one that’s sent out in Nov /Dec that indicates what they get paid starting Jan of incoming year)

Hopefully your State has all this as an online process, so you can scan and email what’s needed. If it doesn’t, I would highly suggest that you BOTH snail mail with the certified mailing card attached to it and then send via a fax from a place that does fax transmission report (date time indicated along with # of pages). Fed Ex stores do this and it’s all DiY. Stuff gets misplaced often and to me this too too important not do what you can to CYA.

If the State is sending this out via snail mail, do look the date of letter inside and the postmarked date on the envelope. For my mom, there was a gap of about 2 weeks on this for the first renewal request. The rub was it all has to be submitted in 30 days from date of letter or they would be suspended from LTC Medicaid the incoming month. By the time I got it and opened it was due in 4 days. A fricking long weekend as I had put all mom’s paperwork up in storage. Had no idea renewals even happened. For year 2, I was more than ready! Personally I think the time lapse is a feature and not a bug.

If Uncle still owns a home, there will be a second mailing sent out that is a “Right of Return” document. It enables the home to remain an exempt asset. fwiw the County / Parish tax assessor/ collector may do their own version of this as well.

The renewal form will tell you what you need. It is usually the most recent bank statememts, pension statement and the SS statement that he gets at the beginning of the year showing what he will be getting for that year. My husband gets his pension statement for tax purposes at the end of the year.

My Mom had no bank acct after spend down. If you took any money out of his acct, you may need to prove it was used for him?
